In our consortium (Erasmus MC, Xpert Clinics, OLVG, Rijndam, Reade, ESHPM) applications of patient and outcome information have been developed for patients after stroke and patients with hand and wrist disorders. This concerns:display of individual patient and outcome information, integrated in the electronic patient recordrepresentation of the personal request for help and the individual treatment goaldisplay of individual predictions of outcomesautomatic identification and feedback of ‘extreme values’
Impact
In this project we use how often these applications are used, and they are for tasks and patients. Also the effects of effects on results, the patient’s insight into the pros and cons of the treatment and expectations.
